Denying this final choice treats desperate people as property of the state rather than agents of their own fate.

Your autonomy argument collapses the moment you apply it consistently. If autonomy is absolute, why do we prevent people from selling themselves into slavery or signing away their organs to the highest bidder? Because autonomy isn't a trump card, it's constrained by what we owe each other as a society.
